Form 15227: Application for an Identity Protection Personal Identification Number (IP PIN)

Form 15227 is the Application for an Identity Protection Personal Identification Number (IP PIN). The form says its primary purpose is to provide a paper method of requesting an Identity Protection PIN. This PDF is the English-Spanish version, Form 15227 (en-sp): page 1 is in English and page 2 carries the same form in Spanish.

The form says the IRS is required to contact the taxpayer applying to validate their identity before issuing the IP PIN, so it asks for a telephone number and a language preference along with a name, mailing address and SSN or ITIN. It also says not to submit it if an IP PIN has already been assigned, and that an IP PIN can be obtained immediately at irs.gov/getanippin.

The completed form goes to the IRS by mail or by fax, not both. You can fill out Form 15227 free in your browser on this page and download it without an account.
