Form 433-B (OIC): Collection Information Statement for Businesses

Form 433-B (OIC), Collection Information Statement for Businesses, is the financial statement a business completes with Form 656, Offer in Compromise. The form says it is for a corporation, a partnership, an LLC classified as a corporation or another LLC, and that it should only be used with Form 656.

It asks for business information and owners, assets, average monthly income and expenses, and other information, then walks through a calculation of a minimum offer amount that goes on Form 656, Section 4, Payment Terms. The form says a sole proprietorship does not use it and completes Form 433-A (OIC), Collection Information Statement for Wage Earners and Self-Employed Individuals, instead.

This is the April 2026 revision (Rev. 4-2026). The PDF can be filled out free in the browser on this page and downloaded without an account.

How to fill out Form 433-B (OIC)

  1. 1

    Gather statements

Section 2 says to gather the most current statements from banks and lenders, loan balances, monthly payments, accountant's depreciation schedules and current values of vehicles and business assets.

  2. 2

    Section 2 assets

Round to the nearest dollar and enter "0" instead of a negative number. For real estate, vehicles and equipment, multiply current market value by .8 and subtract the loan balance. Add lines (1) to (5) in Box A.

  3. 3

    Sections 3 and 4

  4. 4

    Section 5

For 5 or fewer payments within 5 months or less, multiply Box D by 12. For 6 to 24 months, multiply Box D by 24. Add Box A and place the result on Form 656, Section 4.

  5. 5

    Sections 6 and 7

Answer each yes or no question in Section 6 with details for each Yes, then sign Section 7 with a title and date.
