Supporting Documents for Dependents
Form 886-H-DEP, Supporting Documents for Dependents, is a two-page list from the Internal Revenue Service of the documents to send as proof for a dependent claimed on a return. The top of the form has spaces for the taxpayer name, taxpayer identification number and tax year. A table matches each case (a divorced, separated or living-apart parent, a qualifying child, or a qualifying relative) with the photocopies the form asks for.
This revision, Rev. 10-2022, refers to 2022 throughout, for example a child who lived with the taxpayer for more than half of 2022. Page 2 lists documents that show the taxpayer provided more than half of the dependent's total support. The form says the IRS cannot accept documents signed by someone related to the taxpayer.

Add copies from page 2, such as a child support agency statement, rental agreements or utility, repair, school, medical and clothing bills with canceled checks or receipts.
The form asks for photocopies and notes that more than one document may be needed to show the child lived with the taxpayer for more than half of the year.
