Form 9465 (zh-s): Installment Agreement Request, Simplified Chinese Version

Form 9465 (zh-s) is the Simplified Chinese version of Form 9465, Installment Agreement Request. The two-page PDF is titled "分期付款协议申请", and its lines and notes are in Simplified Chinese for people who read Chinese. A few header items, such as "Department of the Treasury" and the OMB number, stay in English. It is dated September 2020 ("2020 年 9 月修订版", Rev. 9-2020), the same revision the IRS lists for the English form.

An individual uses it to ask the IRS to pay tax owed in monthly installments. Part I holds the request: names, address, the amounts owed, a proposed monthly payment, a payment day, and direct debit or payroll deduction options. Part II asks about household, pay and expenses, but only when three conditions on the form all apply. The form says that people who owe $50,000 or less may be able to set up an installment agreement online at www.irs.gov/OPA instead, with a lower user fee.

The instructions are separate, and the form points to English ones: it says to visit www.irs.gov/Form9465 for them and marks that link as English ("英文"). The English edition, Form 9465, has its own page. You can fill out this Chinese PDF free in your browser on this page and download it without an account.

What's on Form 9465 (zh-s)

Part I header: form and periods

The tax form the request covers, for example Form 1040 or Form 941, and the tax years or periods involved.

Lines 1a to 4: who is asking

Names and Social Security numbers, current address with foreign address fields, a box if the address is new, business name and EIN, and home and work phones with the best time to call.

Lines 5 to 10: what is owed

Total owed on the return or notice, other balances due, any payment sent with the request, the balance owed, and that balance divided by 72.0.

Lines 11a to 12: proposed payment

The monthly amount you can pay, a revised amount on line 11b, a box for attaching Form 433-F, and the payment day each month, no later than the 28th.

Lines 13 and 14: how to pay

Routing and account numbers for direct debit with an authorization, a box on line 13c for low-income taxpayers, and a box for payroll deduction with Form 2159.

Part II: additional information

Lines 15 to 27 ask about county of residence, marital status, dependents, people 65 or older at home, pay frequency and net pay, vehicles and car loans, health insurance, court-ordered payments and child or dependent support.

How to fill out Form 9465 (zh-s)

  1. 1

    Top of Part I

Enter the tax form, such as Form 1040, and the tax years or periods the request covers.

  2. 2

    Lines 1a to 4

Enter names and Social Security numbers in the order shown on the return, the current address and phone numbers. Check line 1b if the address is new since the last return.

  3. 3

    Lines 5 to 10

Enter the total owed from the return or notice on line 5 and any other balances on line 6. Add them on line 7, subtract any payment on line 8 to get line 9, and divide line 9 by 72.0 on line 10.

  4. 4

    Lines 11a and 11b

Enter the monthly amount you can pay. The form says that with no amount, the payment is set by dividing line 9 by 72 months. If line 11a is below line 10, raise it on line 11b, or check the box and attach Form 433-F.

  5. 5

    Lines 12 to 14

Choose a payment day no later than the 28th. For direct debit, enter the routing and account numbers on lines 13a and 13b. For payroll deduction, check line 14 and attach Form 2159.

  6. 6

    Sign

Sign and date. On a joint return both spouses sign. The form says signing lets the IRS contact third parties and disclose tax information to process and manage the agreement.

Where it goes and what to know

With your return

The form says it can be attached to the front of the tax return when filed together. The instructions say to send it to the address in the return booklet.

By itself

If the return is already filed, or the form answers a notice, the instructions say to file it alone at the IRS address in their table, which depends on where you live and whether the return has Schedule C, E or F.

Online option

The form says people who owe $50,000 or less may set up an agreement online at www.irs.gov/OPA, with a lower user fee than with Form 9465.

Form 433-F

The form says to complete and attach Form 433-F if line 9 is more than $50,000, or if the payment cannot be raised to the line 10 amount.

Response time

The instructions say the IRS usually replies within 30 days, or longer for tax due on a return filed after March 31.

Canceling a direct debit

The authorization on line 13 says to call 1-800-829-1040 at least 14 business days before the payment date to revoke a payment.

Who fills out Form 9465 (zh-s)?

  • Individuals who owe income tax

The instructions say people who owe income tax on Form 1040 or 1040-SR can use it.

  • People liable for a trust fund recovery penalty

The instructions list individuals who are or may be responsible for a trust fund recovery penalty.

  • Owners of a closed sole proprietorship

Line 2 asks for the business name and EIN and notes that the business is no longer operating. The instructions cover employment taxes owed from such a business.

  • Joint filers

Line 1a has spaces for a spouse's name and Social Security number, and the form says both spouses sign on a joint return.

  • When the instructions say not to use it

If the full amount can be paid within 180 days, if the plan is requested online, or if a business still operating owes employment or unemployment taxes.

  • Readers of Chinese

Line labels and notes are in Simplified Chinese. The IRS also lists a Traditional Chinese edition, a Spanish edition and the English Form 9465.

The individual who owes the tax completes and signs it, and sends it to the IRS. On a joint return both spouses sign. The English instructions list who can use the form.
