Blanket Purchase Agreement
GSA Form 3521, Blanket Purchase Agreement, is a General Services Administration form. Its header names the General Services Administration, and its footer reads GSA FORM 3521 (REV 4-92). GSA hosts it in its forms library, and GSA's page lists APD 2800.12B as the authority.
The two-page agreement is a letter signed by a contracting officer to a supplier. The form says the supplier agrees to furnish the supplies or services described, if and when ordered by the contracting officer or an authorized representative during the term. It sets out pricing, a purchase limit, the GSA employees authorized to place orders and their dollar limits, how orders are placed, what delivery tickets show and where invoices go. The supplier accepts by signing and returning a copy. The form prints no OMB number and no privacy statement.

Purchases are based on a written quotation in response to Standard Form 18, Request for Quotation, or an oral quotation. The form says any one purchase shall not exceed $10,000, and lower amounts may be set for named individuals.
