Optional Form 1419 (OF 1419): Abstract of Offers - Construction

Optional Form 1419 (OF 1419), Abstract of Offers - Construction, is a one-page form for recording the offers received in response to a construction solicitation. GSA hosts it in its forms library. The edition printed on the form is OPTIONAL FORM 1419 (11-88), and the footer says the form is authorized for local reproduction and the previous edition is usable.

The form records the solicitation number, the dates issued and opened, the issuing office, the project title and the number of amendments issued. Each line item has a description, an estimated quantity and a Government estimate, and the offer columns record each offeror's bid security, amendments acknowledged, unit prices and estimated amounts. A certifying official signs that all offers received were opened, read and recorded.

The form holds two offers. Its note says that if more than two offers are received, continue on OF 1419A. You can fill it out free in your browser on this page and download the result without an account.
