Application for Waiver of Passport and/or Visa
Form I-193, Application for Waiver of Passport and/or Visa, is a one-page Department of Homeland Security form. The USCIS page describes it as the form for an alien who is a legal resident of the United States to reenter the United States without the alien's passport and/or visa.
The applicant gives identity, destination, arrival, visa and passport details, then explains in Item 10 why the passport, the visa or both are not in their possession. The bottom of the form is for the office that decides the application, which records approval of a waiver under section 211(b) or section 212(d)(4), or disapproval.
The USCIS page lists no separate instructions document. Payment instructions are printed on the second page of the form itself.
Name, United States destination address, permanent address abroad, country of citizenship or nationality, and place and date of birth.
Date of arrival, port of arrival and manner of arrival, such as the name of the vessel or airline.
Where a visa was previously issued, with its date, number, classification and validity, and where the passport was issued, with its date, number and validity.
Why the applicant is not in possession of the passport, the visa or both, continued on the reverse if necessary.
Date of the application, city and state, the applicant's signature certifying the answers are true and correct, and the preparer's declaration when someone else prepared it.
Marked for the applicant not to write below the line. The office records approval of the waiver under section 211(b) or 212(d)(4), the nonimmigrant class and date admitted until, or disapproval.
The USCIS page says a new edition will be published soon and that in the meantime the 03/01/24 edition may still be used despite the expiration date printed on it.
Enter dates of birth in mm/dd/yyyy form, as the form shows, and give the visa and passport details asked for in Items 8 and 9.
Check passport, visa or both, then write the reason. The form says to continue on the reverse if necessary.
The form marks the bottom area Applicant, do not write below this line. That section is completed by the office that acts on the application.
The applicant signs to certify the answers are true and correct. The USCIS page says an unsigned form is rejected and returned. This page lets you fill the form online free, then print it for signature.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The USCIS page lists the local office with jurisdiction over the port of entry where the alien is entering. It lists no online or lockbox filing option for this form.
The USCIS page points to its Fee Schedule page (Form G-1055) for the fee, which can change. Check it on the day you file. The form says the fee cannot be refunded regardless of the action taken.
The form says not to mail cash, to pay the exact amount, and to make a check or money order payable to U.S. Customs and Border Protection or Department of Homeland Security, except in Guam and the U.S. Virgin Islands. Some filing locations accept credit cards.
Personal checks and money orders are drawn on a U.S. institution in U.S. currency. The form says an uncollected check makes the application and any document issued under it invalid, and a check drawn on another person's account carries the applicant's name.
The USCIS page says unsigned forms are rejected and returned, and that a form with missing pages or pages from a different edition may be rejected.
