Sponsor's Notice of Change of Address
Form I-865, Sponsor's Notice of Change of Address, is how a sponsor reports a new address or residence to USCIS. The instructions say a person who at any time in the past completed Form I-864, Affidavit of Support, to sponsor an immigrant reports a change of address within 30 days of the change if the sponsorship agreement is still in force.
The instructions say the agreement stays in force until the sponsored immigrant becomes a U.S. citizen, receives credit for 40 quarters of work, departs permanently and abandons or is held to have abandoned permanent resident status, loses that status in a removal proceeding, or dies.
The USCIS page lists edition 11/10/20 and says USCIS will also accept the 07/17/15 edition. The form is filed by mail or courier to a USCIS address in Minneapolis.
Full name, date of birth, the new physical address and its effective date, whether it matches the mailing address, and the new mailing address and its effective date.
Names and A-Numbers, if any, for up to four sponsored immigrants, with more listed in Part 6.
Whether the sponsor read the notice or used an interpreter or preparer, contact information, the certification and the signature.
Contact details, certifications and signatures for anyone who interpreted or prepared the notice. The instructions say an attorney or accredited representative also submits Form G-28.
Extra space for longer answers or additional sponsored immigrants, with page, part and item number columns.
The instructions say not to give a PO Box for the physical address. Add the date the change became effective in mm/dd/yyyy form.
Items 6.a. to 7. are for a new mailing address. The instructions say they can be skipped when the mailing address is the same as the new physical address.
Give each person's name and A-Number, if any, in Part 2. More than four go in Part 6 with the same information.
Type or print in black ink. Write N/A where a question does not apply and None for a zero, and put your name on any extra sheet with the page, part and item number.
A stamped or typewritten name is not accepted, and the USCIS page says an unsigned form is rejected and returned. A parent or legal guardian may sign for a sponsor under 14. This page lets you fill the form online free, then print it for signature.
The instructions suggest keeping a copy of the completed form and proof it reached USCIS, such as a USPS certified mail receipt with a return receipt, or a courier shipping label with signature proof of delivery.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The USCIS page gives one address for U.S. Postal Service, FedEx, UPS and DHL deliveries: USCIS, Attn: Form I-865, in Minneapolis, Minnesota. Check the page for the full street address before mailing.
The instructions say within 30 days of the change of address, for as long as the sponsorship agreement is in force. Permanent resident sponsors also have the 10-day address change rule in 8 CFR 265.1.
The USCIS page lists edition 11/10/20 and says USCIS will also accept the 07/17/15 edition. Keep the edition date and page numbers visible on every page, all from the same edition.
The fee is on the USCIS Fee Schedule page and in Form G-1055, which can change. Check it on the day you file.
The instructions say a sponsor who does not give notice of a change of address may be liable for a civil penalty under 8 U.S.C. 1183a(d)(2), and that the amount depends on whether the sponsor knew the immigrant received means-tested public benefits.
