Information on Certain Shareholders of an S Corporation
Schedule B-1 (Form 1120-S) is Information on Certain Shareholders of an S Corporation. The instructions say to use it to provide the information applicable to any shareholder in the S corporation that was a disregarded entity, a trust, an estate, or a nominee or similar person at any time during the tax year.
The S corporation completes the schedule and attaches it to Form 1120-S. It is not filed by the shareholders themselves. The instructions say all S corporations that answer Yes to Form 1120-S, Schedule B, question 3 file it. The schedule has one table and its instructions are printed on page 2 of the same PDF.
You can fill out Schedule B-1 (Form 1120-S) free in your browser on this page and download it without an account.

Name and SSN or EIN, if any, of the individual or entity responsible for reporting the shareholder's income, deductions, credits, etc., from Schedule K-1.

It depends on the type of trust. A grantor trust lists the grantor, a QSST lists its individual beneficiary, and an ESBT repeats columns (a) and (b) with no beneficiary information.
The instructions say to enter the information from columns (a) and (b) and not to enter any beneficiary information.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The schedule says to attach it to Form 1120-S. It prints no separate due date or mailing address.
The table heading points to Form 1120-S, Schedule B, Question 3, and the instructions tie filing to a Yes answer there.
The instructions say to provide the information even when the owner's or the beneficial holder's name, address, and SSN or EIN were already entered on Schedule K-1 (Form 1120-S).
The schedule points to www.irs.gov/Form1120S for the latest information. The Paperwork Reduction Act Notice is in the Instructions for Form 1120-S.
