Form 14568: Model VCP Compliance Statement

Form 14568 is the Model VCP Compliance Statement, used with a Voluntary Correction Program (VCP) submission for a retirement plan. It states that the IRS will not pursue revocation of the tax-favored status of the plan under IRC Section 401(a), 403(b), 408(k) or 408(p) on account of the failures described, and that the statement is conditioned on completing all corrections within 150 days of its date.
